If 4 units or more are required, 5ml of patient's blood in a plain sterile tube and 2ml in
EDTA must be sent with the requisition form. |

If less than 4 units are required, 5ml of patients blood in a plain sterile
tube must be sent with the requisition form. |

In a new born baby upto 6 months old, send the mother's blood sample also. |
| 4. |
The requisition form must be filled complete in all respects. Patient's name, Hospital,
Admn. No., room No. and Ward (Bed) No. should be the same both on the form and the label
on the blood sample. It should be legible otherwise requisition will not be accepted. |

Booking will be cancelled after 3 days unless M.O.I/c of the patient
informs the next date on which it will be required. |
| 6. |
A new blood sample of the patient is required for cross matching if earlier transfusion
was given more than 3 days back. |
| 7. |
Requisitons are accepted round the clock. |
| 8. |
Products once issued will not be taken back in the blood bank. |
| 9. |
The form can be submitted online or printed and faxed. When sending online, kindly ensure that all required fields are filled in complete. When printed and faxed, the page should contain the signature and stamp of the Hospital.
